Report a Provider or Hospital:

This form collects feedback from North Carolina doulas to address policies at perinatal care locations that impact doulas and their clients. The NCDO will use this information to identify trends in different areas or organizations that must be addressed.

Thank you for sharing this information. If a call to action is needed, the NCDO will contact you.

Please DO NOT share any private information about the families you serve that would violate their privacy or HIPPA. Ex: birthdates, age, SS#, etc.

Be sure to receive consent from your families to share their stories.
